Bipolar disorder

Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in a person’s mood, energy, and ability to function. Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide.

Bipolar disorder causes dramatic mood swings—from overly “high” and/or irritable to sad and hopeless, and then back again, often with periods of normal mood in between. Severe changes in energy and behaviour go along with these changes in mood. The periods of highs and lows are called episodes of mania and depression.

Signs and symptoms of the depressive phase of bipolar disorder include: persistent feelings of sadness, anxiety, guilt, anger, isolation and/or hopelessness, disturbances in sleep and appetite, fatigue and loss of interest in usually enjoyed activities, problems concentrating, loneliness, self-loathing, apathy or indifference, depersonalization, loss of interest in sexual activity, shyness or social anxiety, irritability, chronic pain (with or without a known cause), lack of motivation, and morbid/suicidal ideation. In severe cases, the individual may become psychotic, a condition also known as severe bipolar depression with psychotic features.

In some people, however, symptoms of mania and depression may occur together in what is called a mixed bipolar state. Symptoms of a mixed state often include agitation, trouble sleeping, significant change in appetite, psychosis, and suicidal thinking. A person may have a very sad, hopeless mood while at the same time feeling extremely energized.

Both children and adolescents can develop bipolar disorder. It is more likely to affect the children of parents who have the illness. Unlike many adults with bipolar disorder, whose episodes tend to be more clearly defined, children and young adolescents with the illness often experience very fast mood swings between depression and mania many times within a day. Children with mania are more likely to be irritable and prone to destructive tantrums than to be overly happy and elated. Mixed symptoms also are common in youths with bipolar disorder. Older adolescents who develop the illness may have more classic, adult-type episodes and symptoms.

The mainstay of treatment is a mood stabilizer medication; these comprise several unrelated compounds which have been shown to be effective in preventing relapses of manic, or in the one case, depressive episodes. The first known and "gold standard" mood stabilizer is lithium, while almost as widely used is sodium valproate, originally used as an anticonvulsant. Other anticonvulsants used in bipolar disorder include carbamazepine, reportedly more effective in rapid cycling bipolar disorder, and lamotrigine, which is the first one to be shown to be of benefit in bipolar depression.

Treatment of the agitation in acute manic episodes has often required the use of antipsychotic medications, such as Quetiapine, Olanzapine and Chlorpromazine. More recently, Olanzapine and Quetiapine have been approved as effective monotherapy for the maintenance of bipolar disorder. A head-to-head randomized control trial in 2005 has also shown olanzapine monotherapy to be as effective and safe as lithium in prophylaxis.

The use of antidepressants in bipolar disorder has been debated, with some studies reporting a worse outcome with their use triggering manic, hypomanic or mixed episodes, especially if no mood stabiliser is used. However, most mood stabilizers are of limited effectiveness in depressive episodes.

Anticipation of U.S. government data

Its an exciting day in the exchange market today. The market went into the total extreme in anticipation to the U.S. government data that is to be release today at 10:30 am New York time on Wednesday.

The American greenback had slide to an all time low against major currencies. The Euros against the USD, trading at around 1.5060 in Singapore time 10:20pm. 1 British Pound can buy you around 1.9850 USD at the same time.

Further rate cuts had added the pressure for the USD to slide further. Moreover weak data such as a drop in consumer confidence and a higher producer price shown in January are putting more pressure on the dollar. Gloomy outlook for the U.S. economy further worsen the situations.

Fall in the dollar had made investor looking elsewhere to put their money into. Taking the led is Oil, traditionally seen as the black gold. It had hit a record of above 102 USD per barrel. Gold traditionally view as the safe haven for investor was also making the news on record breaking. It is now trading at around 959.80 per ounce at 10:35 pm Singapore time.

I could not slp tonite. I dunno why :/ still very wide awake. forcing myself to slp is not very possible (i laid in bed for more then hr)

Anyway i had some family dinner today. My Bro-in-law and Sis brought dabao. We had Curry fishhead, bbq Sotong, fried bean sprout and rojak. The curry fishhead and bbq sotong were acceptable. The Taste were still good after the long 'duration' in the 'pack'. The bean sprout and rojak were a total disaster. It had become soggy. The rojak was the worst loh. It taste as if it was soaked in some soap water.

My little nephew got none of the food (haha). He got no teeth and those food were mixed with chilli so too bad :D. He is making much progress also. At 8 months old, he is able to crawl all over the place and standing for him is of no problem with some aid.

I got a sudden call from BaoJie today!!! She is still not yet working loh. We had some chat. SHe seems fine and is not in a hurry to find a job (envy leh). she told me something very odd but i think she had misunderstood it. It was not a bad thing, it was a good one and in fact i actually wished it happened to be like what she said. But i was too late..

1 week into the break. Lesson's not going to start anytime soon. Schedule's confirmed and 7th March's when the first lesson starts. With so much time on hand, i got a bit bored. Nothing much anyway also except work.
